Intake form must be completed at least 24 hours in advance
✔ After completing the intake, available dates and times will be shared to schedule an appointment

NO INTAKE = NO SERVICE

Required at Appointment:
• Valid ID
Medical Insurance Card

Individuals without a completed intake form and required documents will not be assisted.

Clients may qualify to receive a free cell phone through government-supported programs that help low-income individuals stay connected. Our team assists with the phone set-up, verifies eligibility, and ensures clients can access essential communication for work, healthcare, housing, and emergency needs.

The Giving Closet provides clients access to low-cost items from our thrift store, including clothing, household essentials, and personal-care items. This program supports individuals and families in meeting their basic needs with dignity while staying within their budget.
