Group Appointment Requests
If you are a case manager, hospital, clinic, shelter, or other similar entity seeking to bring your clients to receive our services, please complete the below form to book a one-day appointment.
When making your request, kindly include:
The number of clients you would like to book
At least three possible dates and times for the appointment
Important:
Every client must
bring a completed application
A valid photo ID is required (international IDs accepted)
Clients must provide their Medical Plan card
