Read before booking an appointment
✅ Please complete the below form with your household details and provide consent where indicated.
✅ Have identification and Medi-Cal plan cards available for all household members.
✅ Please note that we are unable to see clients who do not submit their medical CIN number and a copy of their medical card through the intake form prior to the appointment. If this information is not provided, the appointment will be automatically cancelled.
✅ Clients who miss their virtual or in-person appointment without attending may not be eligible to schedule future appointments. If you are unable to attend, we strongly recommend rescheduling at least 6 hours before your scheduled appointment time.
✅ You will be able to select an appointment after completing your intake. Once you have scheduled your appointment, click “Schedule Appointment”, and then, click “Submit.”
✅ For housing services, the form takes approximately 10 minutes, plus 2 additional minutes per household member. For other services, completion typically takes less than 5 minutes.
