Visit Appointments

Read before booking an appointment

✅ Complete the intake form with your household details and provide consent where required.
✅ Have IDs and Medi-Cal cards ready for all household members.
✅ Medical Info Required: Submit your CIN and medical card in the intake form before your appointment or it will be canceled.
✅ Missed Appointments: No-shows may not be able to schedule again. Please reschedule at least 6 hours in advance.
✅ Time: Housing forms take ~10 minutes (+2 per member); other services take under 5 minutes.
